If you are a new patient, please do the following:1) Obtain a referral (if needed) from your primary care doctor (PCP).
2) Gather all reports, test results, and films relevant to your problem.
3) Download, print out, and fill in the Initial Visit Form found below. Please note it is in Adobe format and you must have Adobe Reader to view and print it.
4) Take all the above with you to your appointment.
